The predictive value of an acute octreotide suppression test in patients with acromegaly

چکیده
Acromegaly is a rare chronic disorder that is in 95% of cases caused by growth hormone (GH) secreting pituitary tumors. Endonasal transphenoidal surgery is usually considered the first line of treatment, followed by medical therapy for residual disease. The long-acting somatostatin (SA) analogues have an important role in the medical treatment of these patients. SA exert their biological effects by activating somatostatin receptors (SSTR). The predominant types of SSTR receptors in GH-secreting pituitary tumors are subtypes 2 (SSTR2) and 5 (SSTR5). The efficacy of somatostatin analog therapy (SAT) is determined by its effect on tumor shrinkage which is reported to be between 20 and 50%. Approximately 10-30% of GH-secreting pituitary tumors are resistant to SA because of variable or reduced tumoral expression of

Limited predictive value of an acute test with subcutaneous octreotide for long-term IGF-I normalization with Sandostatin LAR in acromegaly.
OBJECTIVES To study whether the growth hormone (GH) response after the subcutaneous administration 50 microg of octreotide (acute octreotide test) has any predictive value for long-term IGF-I normalization with Sandostatin LAR. DESIGN Twenty four therapy-naive patients with active acromegaly were studied. RESULTS > 75% GH decrease in the acute octreotide test predicted long-term IGF-I norma...
متن کاملA single-dose comparison of the acute effects between the new somatostatin analog SOM230 and octreotide in acromegalic patients.
Treatment with the somatostatin receptor (sst) subtype 2 predominant analogs octreotide and lanreotide induces clinical and biochemical cure in approximately 65% of acromegalic patients. GH-secreting pituitary adenomas, which are not controlled, also express sst(5). We compared the acute effects of octreotide and SOM230, a new somatostatin analog with high affinity for sst(1,2,3,5) on hormone r...
متن کاملThe Combination Therapy with Bromocriptine and Cyproheptadine in Patients with Acromegaly
The therapeutic efficacy of the combination of cyproheptadine and bromocriptine was studied in 15 patients with active acromegaly showing incomplete GH suppression in response to bromocriptine therapy alone. The mean basal plasma GH was 31.3±5.5 µg/L, and it decreased to 19.0±3.9 µg/L during the single bromocriptine therapy (10-20 mg for 2 to 21 months). When cyproheptadine (12 to 16 mg for 8 t...
متن کاملLong-Term Remission of Acromegaly after Octreotide Withdrawal Is an Uncommon and Frequently Unsustainable Event.
BACKGROUND Long-term remission of acromegaly after somatostatin analog withdrawal has been reported in 18-42% of patients in studies with a relatively small number of patients using different inclusion and remission criteria. The objectives of this study were to establish the probability and predictive factors for short- and long-term remission [normal IGF-1 for age/sex: IGF-1 ≤1.00 × upper lim...
متن کامل[Evaluation of the efficacy of Octreotide LAR in the treatment of acromegaly--a yearly observation].
Since acromegaly is associated with high rates of comorbidities and increased mortality risk compared to the general population, over the last few years somatostatin analogues have been used to treat acromegaly patients who, following surgery, have not fulfilled cure criteria (basal hGH < 2.5 ng/ml, IGF-1 below normal ranges for age and sex and hGH < 1.0 ng/ml in the 120th min of the OGTT test)...
